A before-and-after image of a patient went viral on X and has already amassed more than 27 million views, but what draws the most attention is not just the result: many people simply do not believe it is real.
The procedure was performed by surgeon Dr. Farzan Malekzadeh, who presented the case as “Picasso Technique 5”, describing a complete facial design carried out in four stages.

The patient, identified as Jacqueline, appears tagged in the original post solely for medical documentation purposes.
The transformation sparked a huge discussion on social media. Some users questioned whether a change of this magnitude can be achieved through surgery.
“There is no way, not even physically possible, to achieve this”, one person commented.
Another wrote: “Is that not even your face anymore?”

Precisely because of the disbelief generated by the photographs, the doctor himself decided to include a very clear warning in his post: the image is real and was not generated with artificial intelligence.
Malekzadeh also asked people to avoid mockery or offensive comments toward the patient and explained that the result is not simply the outcome of an isolated procedure, but rather a process of planning, teamwork, and procedures carried out in different stages.
The image continues to divide the internet between those who believe it shows how far cosmetic surgery can go and those who still look at the before and after asking themselves the same thing: Is it really the same person?
