Rich Piana collapsed at his home in Los Angeles on an ordinary day in 2017.
His partner found him unconscious and called emergency services. Doctors induced a coma in an attempt to control the brain swelling, but the damage had already been done: decades of anabolic steroid use had left his heart enlarged and his organs at their limit. Piana, who publicly admitted to using steroids since adolescence, died weeks later at the age of 46.
His case became an uncomfortable symbol in the fitness world: the same body he displayed as an achievement was the one that ultimately failed first. Today, it circulates as a warning among coaches and sports physicians who insist on the same thing: the chemical shortcut comes with a bill that is collected silently, until it is too late to pay it.
